Highly skilled occupational therapists with a passion for home health care are invited to deliver exceptional rehabilitative services to patients in their own residences. As a Home Health Occupational Therapist, youll play a vital role supporting individuals to regain independence, optimize safety, and master the activities of daily living within the comfort of their home setting.
Schedule & Assignment:
- Full-time, 13-week contract
- Monday Friday, 8:00 AM 5:00 PM
- Manage an average caseload of 2532 points per week (approximately 57 patient visits per day)
- Field-based position with daily travel within your assigned territory
Qualifications:
- Current, active Occupational Therapist (OT) license in good standing
- At least one year of recent home health occupational therapy experience required
- Proficiency with OASIS assessment tool and hands-on documentation in Home Care Home Base (HCHB) strongly preferred
- Effective time management, communication, and clinical assessment skills
- Reliable transportation, valid drivers license, and auto insurance
- Ability to work independently and adapt in a field-based, autonomous environment
Key Responsibilities:
- Conduct comprehensive occupational therapy evaluations and required OASIS assessments
- Develop, implement, and modify individualized care plans focused on ADLs, IADLs, cognitive function, and home safety
- Provide skilled interventions: therapeutic activities, adaptive equipment training, environmental modifications, and patient/caregiver education
- Recommend and assess adaptive/assistive devices and durable medical equipment
- Maintain precise, timely documentation in HCHB
- Collaborate with interdisciplinary team members to ensure comprehensive patient care
- Participate in ongoing case conferences, team meetings, and professional development
