Deliver exceptional care and help patients achieve greater independence as a Home Health Occupational Therapist specializing in home-based therapy. This full-time, 13-week contract role centers on providing expert occupational therapy services directly in patients homes, where your guidance supports functional improvement and long-term safety.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM
• Approximately 2532 points per week (typically 57 patient visits per day, based on visit type)

Key Responsibilities:

• Conduct thorough occupational therapy evaluations, including OASIS assessments for Start of Care, Resumption, Recertification, and Discharge as applicable
• Develop and execute individualized care plans targeting ADLs, IADLs, upper extremity function, cognition, and home safety
• Deliver skilled interventionstherapeutic activities, energy conservation, adaptive and environmental modifications, and DME recommendations
• Assess and educate regarding functional independence, injury prevention, and use of assistive equipment
• Complete precise, timely documentation in Home Care Home Base (HCHB)
• Collaborate with a multidisciplinary team, attending case conferences and contributing to comprehensive care
• Ensure adherence to regulatory, agency, and Medicare home health standards

Qualifications:

• Current, active Occupational Therapist license in good standing
• At least 1 year of recent Home Health OT experience (field experience preferred)
• Proficient with HCHB documentation system
• OASIS proficiency required
• Demonstrated strength in clinical assessment and training
• Reliable transportation, valid drivers license, and auto insurance required
• Strong interpersonal, communication, and organizational skills
