A rewarding opportunity awaits a dedicated Occupational Therapist with a passion for delivering high-quality, patient-centered care in the home health setting. As an integral member of a collaborative healthcare team, you will provide expert therapy services directly in patients homes, championing their independence and enhancing their quality of life through compassionate, evidence-based interventions.
Schedule:
- Full-time, 13-week contract
- Monday Friday, 8:00 AM 5:00 PM
- Approximately 2532 points per week (typically 57 patient visits per day, depending on visit types)
Key Responsibilities:
- Perform comprehensive home health OT evaluations, including OASIS assessments as needed
- Create individualized treatment plans targeting ADLs, IADLs, upper extremity improvement, and overall safety
- Deliver skilled therapy services: therapeutic activities, self-care training, adaptive equipment education, and environmental modifications
- Recommend and assess adaptive or durable medical equipment
- Evaluate ongoing progress, modifying plans and communicating effectively with physicians and interdisciplinary team members
- Educate patients and caregivers on functional skills, safety, independence, and fall prevention
- Document services accurately using Home Care Home Base (HCHB)
- Participate in team conferences, meetings, and professional development sessions
- Maintain compliance with all agency policies and regulatory standards
Qualifications:
- Active and current Occupational Therapist license (in good standing)
- At least 1 year of recent experience in home health occupational therapy (field experience preferred)
- OASIS assessment proficiency required
- Home Care Home Base (HCHB) experience preferred
- Strong clinical, functional assessment, and organizational skills
- Valid drivers license, reliable transportation, and current insurance
- Excellent communication and self-management abilities
