A Home Health Occupational Therapist (OT) is needed to provide expert occupational therapy services directly to patients in their homes. This role focuses on enhancing patients' independence and safety by improving their ability to perform activities of daily living (ADLs). The position requires a self-motivated and organized individual comfortable with working autonomously within a field-based environment.
Key Responsibilities:
- Conduct comprehensive OT evaluations, including OASIS assessments when required.
- Develop and implement individualized care plans focusing on ADLs, upper extremity function, cognition, and home safety.
- Provide skilled interventions such as therapeutic activities, self-care training, energy conservation techniques, adaptive equipment training, and environmental modifications.
- Assess and recommend appropriate durable medical equipment (DME).
- Monitor patient progress and modify treatment plans accordingly.
- Educate patients and caregivers on functional independence, fall prevention, and compensatory strategies.
- Maintain accurate documentation using Home Care Home Base (HCHB).
- Collaborate with interdisciplinary team members for coordinated care.
- Participate in case conferences and ongoing professional development.
- Adhere to agency policies, regulatory standards, and Medicare guidelines.
Qualifications:
- Active Occupational Therapist license in good standing.
- Minimum one year recent experience in Home Health Occupational Therapy; field experience preferred.
- Proficiency with Home Care Home Base (HCHB) and OASIS assessments.
- Strong clinical assessment and functional training skills.
- Reliable transportation, valid drivers license, and auto insurance.
- Excellent communication, organizational, and time management abilities.
Contract Details:
- Full-time, 13-week contract.
- Monday through Friday, 8:00 AM 5:00 PM.
- Caseload of approximately 2532 points weekly (57 patient visits daily).
- Field-based setting with daily travel to patient homes; mileage reimbursement provided.
