Delivering personalized occupational therapy in the comfort of patients homes, this role is ideal for clinicians who are motivated, adaptable, and passionate about helping clients regain independence. As an Occupational Therapist specializing in home health services, youll conduct thorough evaluations, develop targeted treatment plans, and empower individuals to thrive in their daily routines.
Schedule:
- Monday through Friday, 8:00 AM 5:00 PM
- Full-time, 13-week contract
Key Qualifications:
- Active Occupational Therapist (OT) license in the state of practice and in good standing
- At least 1 year of recent home health occupational therapy experience
- Experience with OASIS assessments
- Proficiency with Home Care Home Base (HCHB) preferred
- Strong assessment, functional training, and patient education skills
- Reliable transportation, valid drivers license, and current auto insurance
- Excellent independent work ethic, time management, and communication abilities
Primary Responsibilities:
- Perform comprehensive OT and OASIS assessments in patient homes
- Develop and implement individualized plans of care emphasizing ADLs, IADLs, home safety, and upper extremity function
- Deliver skilled interventions: therapeutic activities, adaptive equipment training, environmental modifications, and safety strategies
- Document all intervention and progress accurately and promptly in HCHB
- Maintain effective communication with physicians and the interdisciplinary team
- Educate patients and caregivers on strategies for functional independence and fall prevention
- Attend case conferences and interdisciplinary meetings, contributing to team-based care
- Ensure all interventions align with agency policies, Medicare requirements, and regulatory standards
