Job Description
Delivering high-quality care in the comfort and independence of patients homes, this field-based position is ideal for an experienced and compassionate Occupational Therapist. You will play a key role in evaluating and treating adult patients who require home-based rehabilitation, focusing on fostering everyday independence, safety, and quality of life.
Schedule:
- Full-time, 13-week contract
- Monday through Friday, 8:00 AM 5:00 PM
- Approximately 2532 points per week (about 57 patient visits per day, based on visit types)
Qualifications:
- Active Occupational Therapist (OT) license in good standing for the state
- At least 1 year of recent home health Occupational Therapy experience, with strong field skills preferred
- Proficiency with OASIS assessments required; familiarity with Home Care Home Base (HCHB) preferred
- Adept at clinical assessments, adaptive equipment recommendations, and functional training
- Reliable transportation, a valid drivers license, and auto insurance are essential
- Self-motivated, organized, and able to thrive both independently and as part of a multidisciplinary care team
Key Responsibilities:
- Conduct thorough occupational therapy evaluations, including OASIS compliance for new and ongoing episodes of care
- Design and deliver tailored care plans that promote self-care, mobility, cognition, and safety in the home setting
- Educate patients and caregivers on fall prevention, adaptive equipment, and home modifications
- Communicate effectively with physicians and other care providers to ensure coordinated, patient-centered care
- Submit timely and precise documentation using HCHB
- Attend interdisciplinary meetings and case conferences as needed
