Deliver high-impact care by supporting patients independence and well-being in their own homes as a dedicated Home Health Occupational Therapist. This contract position emphasizes patient-focused therapy with a collaborative, interdisciplinary approach in a dynamic field-based environment.

Schedule:

• Full-time, 13-week contract
• Monday Friday, 8:00 AM 5:00 PM

Required Qualifications:

• Active Occupational Therapist (OT) license, in good standing
• At least 1 year of recent Home Health Occupational Therapy experience (field-based experience preferred)
• Proficiency with Home Care Home Base (HCHB) is a plus
• OASIS assessment experience required
• Strong clinical assessment and patient training abilities
• Excellent communication, organizational, and time management skills
• Reliable transportation, current drivers license, and auto insurance
• Ability to work autonomously and efficiently manage field assignments

Key Responsibilities:

• Perform comprehensive OT evaluations and OASIS assessments for Start of Care, Recertification, and Discharge
• Develop and implement individualized care plans focused on ADLs, IADLs, and patient safety
• Deliver skilled interventions including therapeutic activities, adaptive and durable medical equipment training, energy conservation, and environmental modifications
• Track patient progress and adjust treatment strategies in collaboration with physicians and the care team
• Provide patient and caregiver education on safety, independence, and use of assistive devices
• Accurately complete timely documentation within Home Care Home Base
• Actively participate in interdisciplinary case conferences and meetings
• Maintain compliance with agency, regulatory, and Medicare requirements

$2,299 - $2,432/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
