Job Description

Bring your expertise in home health occupational therapy to a contract role dedicated to making a meaningful impact on patient independence and quality of life. This field-based opportunity allows you to provide individualized care directly in patients homes, working autonomously while staying connected to a collaborative interdisciplinary team. Youll utilize your clinical skills and compassion to enhance patient outcomes, focusing on ADLs, safety, and adaptive strategies.

Schedule:

  • Full-time, 13-week contract
  • Monday Friday, 8:00 AM 5:00 PM

Key Responsibilities:

  • Conduct detailed occupational therapy evaluations, including OASIS assessments for various patient milestones.
  • Create and implement individualized care plans prioritizing ADLs, IADLs, upper extremity rehabilitation, home safety, and cognition.
  • Deliver evidence-based interventions including adaptive equipment training, energy conservation, and environmental modifications.
  • Recommend and coordinate the use of durable medical equipment suited to patient needs.
  • Monitor progress, making necessary adjustments to care plans and communicating updates to physicians and care team members.
  • Educate patients and caregivers on self-care, fall prevention, and safety techniques.
  • Complete clear, timely documentation in Home Care Home Base (HCHB).
  • Foster effective interdisciplinary communication by participating in meetings, case conferences, and ongoing professional development.

Qualifications:

  • Active Occupational Therapist license (state-specific, in good standing)
  • Minimum 1 year of recent home health OT experience; field experience preferred
  • Proficient with HCHB and OASIS assessments
  • Reliable transportation, valid drivers license, and auto insurance
  • Strong organizational, assessment, and communication skills
