Deliver impactful care by helping patients regain independence in their own homes as a Home Health Occupational Therapist. In this essential role, youll bring clinical expertise to individuals needing support with daily living activities, focusing on safety, function, and overall well-being. Working autonomously in the field, youll evaluate patients, design and implement care plans, and educate both patients and families in adaptive strategies and equipment. Your work will directly support stronger patient outcomes and improved quality of life.
Schedule & Assignment:
- Full-time, 13-week contract
- Monday Friday, 8:00 AM 5:00 PM
- Expect a caseload of approximately 2532 points per week (57 patient visits per day, based on visit type)
- Field-based: daily travel to patient homes within your assigned territory
Your Responsibilities:
- Conduct thorough initial and follow-up evaluations, including OASIS assessments
- Develop and adapt individualized care plans to improve ADLs, IADLs, upper extremity use, cognition, and home safety
- Deliver skilled interventions such as therapeutic activities, self-care training, adaptive equipment use, and environment modifications
- Recommend and coordinate adaptive and durable medical equipment
- Communicate progress and care updates with an interdisciplinary care team and attending physicians
- Provide education on functional independence, fall prevention, and safety for patients and caregivers
- Complete documentation using Home Care Home Base (HCHB) in a timely and accurate manner
- Collaborate at case conferences and participate in ongoing professional development
- Ensure strict compliance with agency policies, regulatory guidelines, and Medicare standards
Qualifications:
- Active Occupational Therapist license (in good standing) for the state
- Minimum 1 year of recent, relevant experience in home health OT (field-based work preferred)
- OASIS experience required; Home Care Home Base (HCHB) proficiency preferred
- Valid drivers license, reliable transportation, and current auto insurance
- Excellent assessment, teaching, time management, and communication skills
- Ability to work both independently and as part of an interdisciplinary team
