Job Description
Deliver personalized, high-quality occupational therapy to patients in their homes as part of a dedicated healthcare team. This full-time, 13-week contract position is designed for a skilled and compassionate Occupational Therapist specializing in home health care. Your expertise will help patients regain independence, improve daily function, and enjoy improved safety within their living environments.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
Key Responsibilities:
- Perform comprehensive assessments and OASIS evaluations for patients in their homes.
- Create and implement individualized care plans targeting ADLs, IADLs, upper extremity function, cognition, and safety.
- Deliver hands-on interventions such as therapeutic activities, adaptive equipment education, and energy conservation training.
- Assess the need for and recommend appropriate durable medical equipment.
- Track patient progress, adapt treatment strategies as necessary, and maintain open communication with physicians and the interdisciplinary team.
- Educate patients and caregivers about functional independence, fall prevention, safety, and adaptive strategies.
- Accurately complete documentation using Home Care Home Base (HCHB).
- Collaborate with nurses, physical therapists, speech therapists, social workers, and home health aides.
- Participate in case conferencing and professional development; ensure compliance with regulatory and agency policies.
Qualifications:
- Active Occupational Therapist (OT) license in good standing.
- Minimum of 1 year of recent home health OT experience.
- Proficiency in HCHB preferred; OASIS assessment experience required.
- Excellent clinical, communication, and time management skills.
- Reliable personal transportation, valid drivers license, and auto insurance.
- Ability to work independently on a field-based schedule.
