Job Description

Bring your expertise as a Home Health Occupational Therapist to a rewarding, patient-centered field role. This position focuses on delivering high-quality therapy services that help individuals regain independence and improve their daily living skills in the comfort of their own homes. You'll collaborate with a skilled interdisciplinary care team and play a pivotal role in evaluating needs, designing individualized interventions, and empowering patients and caregivers with crucial education and resources.

Schedule:

  • Monday Friday, 8:00 AM 5:00 PM
  • Full-time, 13-week contract

Desired Qualifications:

  • Current Occupational Therapist (OT) license in good standing
  • At least 1 year of recent Home Health OT experience (fieldwork preferred)
  • Experience with OASIS assessments
  • Familiarity with Home Care Home Base (HCHB) documentation system preferred
  • Reliable transportation, valid drivers license, and current auto insurance
  • Strong time management, communication, and independent organizational skills

Primary Responsibilities:

  • Perform comprehensive OT assessments, including OASIS as required
  • Plan and implement care addressing ADLs, upper extremity function, cognition, safety, and IADLs
  • Utilize skilled interventions: therapeutic activities, self-care education, equipment training, home modifications
  • Assess and recommend appropriate durable medical equipment
  • Monitor progress, update plans, and communicate with care team and physicians
  • Educate patients/caregivers on independence, safety, fall prevention, and adaptive techniques
  • Maintain accurate, timely documentation in HCHB
  • Coordinate care with PT, nursing, speech therapy, social work, and aides
