Job Description

Deliver essential care and empower independence as a Home Health Occupational Therapist serving patients in their own residences. As part of a compassionate healthcare team, you'll play a vital role in helping individuals regain function, improve daily living skills, and maintain a safe, supportive home environment.

Schedule:

  • Full-time, 13-week contract
  • Monday Friday, 8:00 AM 5:00 PM

Role Responsibilities:

  • Conduct thorough occupational therapy evaluations, including completion of OASIS assessments where needed
  • Create and implement tailored care plans to address ADLs, instrumental ADLs, cognitive challenges, upper extremity function, and home safety
  • Deliver skilled, evidence-based interventions such as self-care training, therapeutic activities, adaptive equipment instruction, and energy conservation strategies
  • Recommend and assess needs for durable medical equipment to ensure patient safety and independence
  • Assess progress regularly, modifying care plans and coordinating communication with physicians and fellow clinicians
  • Provide education to patients and caregivers on injury prevention, adaptive strategies, and equipment use
  • Maintain clear, timely documentation using the Home Care Home Base (HCHB) electronic system
  • Participate in interdisciplinary meetings and stay current with professional development opportunities
  • Ensure all services follow agency, regulatory, and Medicare guidelines

Qualifications:

  • Active Occupational Therapist (OT) license, in good standing
  • At least one year of recent experience in home health occupational therapy; field-based experience strongly preferred
  • Proficiency with Home Care Home Base (HCHB) and required OASIS assessment experience
  • Solid clinical assessment and functional training abilities
  • Reliable transportation with proof of insurance and valid drivers license
  • Excellent time management, communication, and organizational skills; ability to thrive independently
