Job Description

Deliver quality care to patients in a home-based setting as an Occupational Therapist, focusing on enhancing independence, safety, and functional ability. This full-time, 13-week contract position offers the opportunity to manage a flexible caseload, provide hands-on therapy, and work independently within an established care team.

Schedule:

  • Monday Friday, 8:00 AM 5:00 PM

Responsibilities:

  • Conduct comprehensive in-home occupational therapy evaluations, including completion of OASIS assessments as required.
  • Create and implement individualized care plans addressing activities of daily living (ADLs), instrumental ADLs, upper extremity function, cognition, and home safety.
  • Deliver skilled therapy interventions, including therapeutic activities, training in self-care, energy conservation, and adaptive equipment use.
  • Assess for and recommend appropriate durable medical equipment and environmental modifications to support patient independence.
  • Regularly monitor and document patient progress using Home Care Home Base (HCHB), adjusting interventions as clinically indicated.
  • Collaborate extensively with a multidisciplinary teamnurses, physical therapists, speech-language pathologists, medical social workers, and home health aidesin order to provide coordinated care.
  • Educate patients and caregivers on adaptive strategies, safety, fall prevention, and maximizing daily function.
  • Maintain compliance with agency protocols, Medicare home health guidelines, and regulatory standards.
  • Attend interdisciplinary meetings, participate in case conferences, and pursue ongoing professional development.

Qualifications:

  • Active Occupational Therapist (OT) license in good standing within the state.
  • Minimum 1 year of recent field-based Home Health Occupational Therapy experience required.
  • OASIS assessment experience required; proficiency with HCHB preferred.
  • Strong clinical assessment, organizational, and communication skills.
  • Reliable transportation, drivers license, and auto insurance.
