Job Description
Deliver meaningful care in the community as a dedicated Home Health Occupational Therapist, serving patients in their homes and helping them regain independence in daily life. This contract opportunity centers on individualized therapy, empowering each patient through expertly developed plans and hands-on interventions. Your clinical expertise and creativity will support functional improvement, safety, and quality of life.
Schedule:
- Monday through Friday, 8:00 AM 5:00 PM
- Full-time, 13-week contract
- Approximately 2532 points per week (typically 57 patient visits per day, depending on visit types)
Desired Qualifications:
- Active Occupational Therapist (OT) license, in good standing
- At least one year of recent home health experience as an OT
- Required proficiency in OASIS assessments
- Familiarity with electronic documentation systems such as Home Care Home Base (HCHB) preferred
- Strong clinical, assessment, and training skills
- Ability to communicate effectively with patients, families, and the care team
- Organized, self-directed, and able to excel in a field-based role
- Reliable transportation, valid drivers license, and auto insurance are essential
Key Responsibilities:
- Complete thorough occupational therapy evaluations, including OASIS where applicable
- Design and execute individualized therapy plans oriented to ADLs, IADLs, upper extremity function, cognition, and safety
- Deliver therapeutic interventions, equipment training, and home modifications
- Recommend and educate on appropriate adaptive and durable medical equipment
- Monitor patient outcomes, modify care as needed, and report to the interdisciplinary team
- Educate patients and caregivers on independence, fall prevention, and compensatory strategies
- Accurately document visits and progress using HCHB system
- Attend interdisciplinary meetings and case conferences to coordinate holistic care
- Maintain compliance with standards and regulatory requirements
