Job Description

Make a direct impact on patient well-being by delivering essential care in a flexible field-based role as a Home Health Occupational Therapist. This full-time, 13-week contract position calls for a self-motivated clinician to provide high-quality therapy services in patients homes, focusing on fostering independence, safety, and improved activities of daily living (ADLs).

Schedule:

  • Monday Friday, 8:00 AM 5:00 PM

Key Responsibilities:

  • Perform comprehensive occupational therapy evaluations, using OASIS assessments for all relevant care episodes.
  • Develop and implement tailored interventions to address ADLs, IADLs, upper extremity function, cognition, and home safety.
  • Deliver hands-on therapy services such as energy conservation, adaptive equipment use, and patient/caregiver education.
  • Assess and recommend appropriate durable medical equipment (DME).
  • Monitor and document patient progress thoroughly in Home Care Home Base (HCHB).
  • Collaborate with an interdisciplinary team, participating in case conferences and ongoing professional development.
  • Drive to patient homes daily within the assigned territory (mileage reimbursement included).
  • Ensure compliance with regulatory standards and Medicare guidelines.

Qualifications:

  • Active Occupational Therapist (OT) license in good standing.
  • At least 1 year of recent home health OT experience; field work preferred.
  • Experience with OASIS and familiarity with Home Care Home Base (HCHB) is highly valued.
  • Strong clinical, assessment, and organizational skills.
  • Ability to work autonomously, including schedule management in the field.
  • Reliable transportation, valid drivers license, and auto insurance.
