Job Description
A rewarding opportunity is available for an experienced Occupational Therapist to deliver specialized home health therapy services. This role focuses on enhancing patients ability to perform activities of daily living (ADLs) and promoting independence and safety within their home environments.
Responsibilities include conducting thorough occupational therapy evaluations using OASIS assessments, developing customized treatment plans addressing ADLs, upper extremity function, cognition, and home safety, and providing skilled interventions such as therapeutic activities, adaptive equipment training, and environmental modifications. The therapist will also assess the need for durable medical equipment, educate patients and caregivers on fall prevention and safety techniques, and collaborate with an interdisciplinary team to ensure comprehensive patient care. Accurate and timely documentation in the Home Care Home Base (HCHB) system is essential.
Key Qualifications and Experience:
- Active and valid Occupational Therapist license in good standing
- Minimum of 1 year recent experience in Home Health Occupational Therapy, with field experience preferred
- Proficiency in OASIS assessments; familiarity with Home Care Home Base (HCHB) preferred
- Strong clinical assessment, functional training, and patient education skills
- Reliable transportation, valid drivers license, and automobile insurance
- Ability to work independently and manage a flexible, field-based schedule
- Excellent organizational, communication, and time management skills
Position Details:
- Contract duration: Full-time, 13 weeks
- Schedule: Monday Friday, 8:00 AM 5:00 PM
- Caseload: Approximately 2532 points weekly (about 57 patient visits daily)
- Work setting: Patient homes within assigned territory; mileage reimbursement provided
