A dedicated Home Health Occupational Therapist (OT) role is available for a motivated professional seeking to deliver compassionate, patient-centered care in home settings. This full-time, 13-week contract position offers the opportunity to enhance patient independence and safety by providing skilled occupational therapy services directly in patient homes.

Location: This position is based in Fountain, CO, a vibrant community known for its beautiful natural surroundings and family-friendly amenities. Residents enjoy close proximity to outdoor recreational opportunities and a welcoming environment that supports a balanced lifestyle.

Schedule & Workload:

• Monday through Friday, 8:00 AM 5:00 PM
• Caseload of approximately 2532 points weekly, translating to about 57 patient visits daily, depending on service needs
• Field-based role requiring daily travel to patient homes within the assigned territory (mileage reimbursement included)

Key Responsibilities:

• Conduct comprehensive OT evaluations including OASIS assessments for start of care, resumption, recertification, and discharge
• Develop and implement individualized treatment plans focusing on activities of daily living (ADLs), instrumental ADLs, upper extremity function, cognition, and home safety
• Provide skilled therapeutic interventions such as adaptive equipment training, energy conservation, and environmental modifications
• Collaborate with interdisciplinary teams to coordinate effective patient care
• Maintain meticulous and timely documentation in Home Care Home Base (HCHB)
• Educate patients and caregivers on safety, functional independence, and compensatory strategies
• Ensure compliance with Medicare home health regulations and agency policies

Qualifications:

• Active Occupational Therapist license in good standing for the state of practice
• Minimum one year of recent home health OT experience, preferably with field work
• Proficiency with OASIS assessments required; experience using Home Care Home Base preferred
• Reliable transportation, valid drivers license, and auto insurance essential
• Strong organizational, communication, and time management skills with the ability to work independently
