Job Description

Deliver exceptional patient-centered care as a Home Health Occupational Therapist specializing in helping individuals achieve greater independence in their daily lives. This 13-week, full-time contract role centers on providing skilled occupational therapy services to patients in their home environments, allowing you to build meaningful relationships while advancing patient outcomes.

Schedule:

  • Monday to Friday, 8:00 AM 5:00 PM

Key Responsibilities:

  • Perform comprehensive evaluations using OASIS assessments when applicable.
  • Develop and implement customized plans of care targeting ADLs, IADLs, upper extremity mobility, cognition, and home safety.
  • Provide skilled interventions, including therapeutic activities, self-care training, adaptive equipment instruction, and environmental modifications.
  • Assess and recommend appropriate adaptive and durable medical equipment.
  • Monitor progress and adapt treatment strategies; communicate effectively with the interdisciplinary care team.
  • Educate patients and caregivers on maximizing independence, fall prevention, and safety techniques.
  • Accurately complete electronic documentation utilizing Home Care Home Base (HCHB).
  • Participate in case conferences, interdisciplinary meetings, and ongoing professional development.
  • Travel daily within an assigned territory, with mileage reimbursement provided.

Qualifications:

  • Active Occupational Therapist (OT) license in good standing for the state of practice.
  • Minimum 1 year of recent Home Health OT experience required.
  • Proficiency with Home Care Home Base (HCHB) preferred.
  • Required experience with OASIS assessments.
  • Reliable transportation, valid drivers license, and insurance.
  • Excellent communication, clinical assessment, organization, and time management skills.
