Job Description
Make a difference delivering personalized patient care as a Home Health Occupational Therapist, focusing on rehabilitation and safety in a patients own environment. This contract opportunity requires a skilled OT to assess, plan, and implement effective therapy interventionsempowering patients to thrive in their daily routines and regain independence.
Schedule:
- Full-time, 13-week contract
- Monday Friday, 8:00 AM 5:00 PM
Key Responsibilities:
- Complete comprehensive occupational therapy evaluations, including OASIS assessments as required.
- Develop tailored therapy goals addressing ADLs, IADLs, upper extremity function, home safety, and cognition.
- Deliver one-on-one care in the patients home, utilizing therapeutic activities, functional training, adaptive equipment instruction, energy conservation strategies, and environmental modifications.
- Recommend durable medical equipment (DME) based on patient assessment, and educate clients and caregivers on its use.
- Continuously assess progress, updating care plans and maintaining communication with physicians and the interdisciplinary team.
- Document visits and progress in Home Care Home Base (HCHB) system accurately and promptly.
- Collaborate with other disciplines including nursing, PT, SLP, MSW, and home health aides to ensure seamless, high-quality care.
- Participate in team conferences, interdisciplinary meetings, and ongoing professional development sessions.
- Adhere to agency policies, regulatory standards, and Medicare guidelines.
Qualifications:
- Active Occupational Therapist (OT) license in the practicing state (in good standing)
- At least 1 year of recent Home Health OT experience required
- Familiarity with OASIS documentation and the HCHB system
- Solid assessment and functional training abilities
- Reliable personal transportation, valid drivers license, and auto insurance
- Comfortable working independently on a flexible, field-based schedule
- Exceptional communication, time management, and organizational skills
