Make a meaningful difference for patients as a Home Health Occupational Therapist, delivering vital therapy services in the comfort of each clients home. In this field-based contract role, youll use your expertise to empower patients to achieve greater independence in daily life and foster safer living environments.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
Core Qualifications:
- Active Occupational Therapist (OT) license in good standing
- At least 1 year of recent home health OT experience (field experience a plus)
- Proficiency with OASIS assessments (required) and experience using Home Care Home Base (HCHB) is preferred
- Outstanding assessment, documentation, and functional training skills
- Excellent interpersonal and communication abilities
- Self-driven with solid time-management and organizational capabilities
- Reliable transportation, current drivers license, and insurance
Key Responsibilities:
- Complete comprehensive OT evaluations, including OASIS assessments where appropriate
- Design and implement personalized care plans that address ADLs, IADLs, cognition, and upper extremity function
- Deliver skilled interventions: therapeutic activities, adaptive equipment training, environmental modifications, and more
- Educate patients and caregivers on functional independence and safety, including fall prevention and compensatory strategies
- Accurately document all care using Home Care Home Base (HCHB) software
- Communicate patient progress and collaborate with interdisciplinary home health team members
- Participate in case conferences and ongoing professional development
- Maintain compliance with agency, regulatory, and Medicare guidelines
