Job Description

Deliver high-quality occupational therapy care and help patients regain independence in their own homes. This role centers on comprehensive assessments, individualized treatment planning, and active collaboration with an interdisciplinary care team for optimal patient outcomes. Your expertise will empower patients to maximize their participation in daily activities, ensure safety, and improve quality of life.

Schedule:

  • Monday Friday, 8:00 AM 5:00 PM
  • Full-time, 13-week contract

What youll do:

  • Provide thorough occupational therapy evaluations, including OASIS assessments for start of care and other milestones.
  • Develop and execute patient-specific care plans focused on ADLs, IADLs, upper extremity function, cognitive strategies, and home safety.
  • Deliver skilled therapy interventions such as therapeutic activities, energy conservation, adaptive equipment instruction, and environmental modifications.
  • Recommend appropriate durable medical equipment and train patients on safe usage.
  • Monitor progress and adjust interventions as needed, communicating updates to physicians and the care team.
  • Educate patients and caregivers on promoting safe, independent living and preventing falls.
  • Maintain accurate, timely documentation using Home Care Home Base (HCHB).
  • Collaborate with nurses, PTs, SLPs, MSWs, and home health aides.
  • Attend case conferences, interdisciplinary meetings, and participate in ongoing professional development.
  • Adhere to policies, regulatory standards, and home health guidelines.

Requirements:

  • Current Occupational Therapist (OT) license (active and in good standing)
  • At least 1 year of recent Home Health OT experience (field experience preferred)
  • OASIS competency required
  • Proficiency with Home Care Home Base (HCHB) preferred
  • Reliable transportation, valid drivers license, and auto insurance
  • Strong clinical, organizational, and communication skills
  • Comfortable working independently in patient homes
