Job Description

A rewarding opportunity is available for a skilled Home Health Occupational Therapist (OT) to provide essential therapy services in a field-based setting. This role focuses on enhancing patients abilities to perform daily activities safely and independently within their homes. Key Responsibilities:

  • Perform comprehensive occupational therapy evaluations, including OASIS assessments.
  • Develop and implement individualized treatment plans targeting ADLs, IADLs, cognition, upper extremity function, and home safety.
  • Deliver skilled interventions such as therapeutic activities, self-care training, adaptive equipment instruction, and environmental modifications.
  • Recommend and assess the need for durable medical equipment (DME).
  • Monitor patient progress, updating care plans and communicating with healthcare providers.
  • Educate patients and caregivers on safety, fall prevention, and compensatory strategies.
  • Maintain accurate documentation using the Home Care Home Base (HCHB) system.
  • Collaborate with nurses, therapists, social workers, and aides to ensure holistic patient care.
  • Participate in case conferences, interdisciplinary meetings, and professional development.
  • Comply with agency policies and Medicare home health guidelines.

Qualifications:

  • Active Occupational Therapist license in Missouri, in good standing.
  • Minimum one year of recent home health OT experience; field experience preferred.
  • Proficiency in Home Care Home Base (preferred) and OASIS assessment experience required.
  • Strong clinical evaluation and functional training abilities.
  • Reliable transportation, valid drivers license, and auto insurance.
  • Ability to work independently with strong communication and time management skills.

Schedule & Contract Details:

  • Full-time, 13-week contract.
  • Monday through Friday, 8:00 AM 5:00 PM.
  • Caseload approximately 2532 points per week (57 patient visits daily).
