Job Description

Empower patients to regain independence by providing vital occupational therapy services directly in their own homes. This full-time, 13-week contract role offers the opportunity to deliver high-quality, patient-centered care as part of an interdisciplinary team.

Schedule:

  • Monday through Friday, 8:00 AM 5:00 PM

Key Responsibilities:

  • Conduct comprehensive in-home occupational therapy evaluations, including OASIS assessments for initial, ongoing, and discharge visits.
  • Design and implement patient-specific plans of care to improve activities of daily living (ADLs), instrumental ADLs, upper extremity function, cognition, and overall home safety.
  • Deliver skilled therapies, including self-care training, therapeutic activities, adaptive equipment instruction, and environmental modifications.
  • Assess and recommend appropriate durable medical equipment to meet patients functional needs.
  • Evaluate progress, update treatment interventions as needed, and maintain ongoing communication with physicians and the home health team.
  • Educate patients and caregivers regarding safety, fall prevention, compensatory strategies, and use of adaptive aids.
  • Complete comprehensive documentation using Home Care Home Base (HCHB) within required timelines.
  • Collaborate during case conferences and interdisciplinary meetings, ensuring cohesive care planning and regulatory compliance.

Qualifications:

  • Active Occupational Therapist (OT) license, in good standing.
  • At least one year of recent field-based home health OT experience.
  • Expertise with OASIS documentation and proficiency with Home Care Home Base (HCHB) preferred.
  • Reliable transportation, valid drivers license, and current auto insurance required.
  • Ability to work independently and navigate a field-based, flexible caseload of approximately 2532 points (57 visits/day).
  • Excellent clinical assessment, communication, and organizational skills.
