Job Description
Deliver high-quality care and make a meaningful difference in patients lives by providing occupational therapy services directly in their homes. This contract opportunity is ideal for skilled and compassionate occupational therapists ready to take on a dynamic, field-based role supporting patient independence and functional improvement.
Schedule & Assignment Details:
- Monday Friday, 8:00 AM 5:00 PM
- 13-week contract term
- Caseload: Approximately 2532 points per week (typically 57 patient visits per day, depending on visit types)
- Patient care provided in-home within assigned travel territory
- Daily travel with mileage reimbursement
- Documentation platform: Home Care Home Base (HCHB)
- OASIS assessment tool utilized for designated patient encounters
Key Responsibilities:
- Perform comprehensive occupational therapy evaluations, including OASIS assessments as needed
- Develop and implement individualized care plans to target ADLs, IADLs, safety, and independence
- Deliver skilled interventions: therapeutic activities, self-care training, energy conservation, adaptive equipment instruction, environmental modifications, and more
- Assess for and recommend durable medical equipment as appropriate
- Monitor and document patient progress using HCHB, making adjustments to care plans to meet evolving needs
- Educate patients and caregivers on functional strategies, safety, and the use of adaptive equipment
- Collaborate closely with interdisciplinary team members for coordinated care delivery, participating in case conferences and ongoing professional development
- Maintain compliance with agency, Medicare, and regulatory guidelines
Qualifications:
- Active and unrestricted Occupational Therapist license
- At least 1 year of recent Home Health OT experience required
- Proficiency with OASIS assessments and HCHB preferred
- Reliable transportation, drivers license, and insurance
- Excellent assessment, documentation, and communication skills
- Ability to independently manage a full, flexible field-based schedule
