Job Description

Provide expert occupational therapy in a home health setting, supporting patients in achieving independence and safety in their daily lives. This full-time, 13-week contract position is ideal for a skilled Home Health Occupational Therapist who excels in field-based care and collaborative clinical environments.

Schedule:

  • Monday to Friday, 8:00 AM 5:00 PM

Key Responsibilities:

  • Conduct thorough occupational therapy evaluations in patients homes, including required OASIS assessments.
  • Develop and implement care plans tailored to improve ADLs, instrumental ADLs, upper extremity function, cognition, and home safety.
  • Deliver skilled interventions such as therapeutic activities, adaptive equipment training, energy conservation, and environmental modifications.
  • Assess the need for adaptive or durable medical equipment and make effective recommendations.
  • Monitor patient progress, adjust plans as needed, and communicate updates with physicians and interdisciplinary teams.
  • Provide patient and caregiver education on functional independence, fall prevention, and use of assistive devices.
  • Maintain accurate, timely documentation using Home Care Home Base (HCHB).
  • Participate in case conferences, interdisciplinary meetings, and ongoing professional development.
  • Maintain compliance with agency, regulatory, and Medicare guidelines.

Qualifications:

  • Current Occupational Therapist (OT) license (state specific, in good standing)
  • At least 1 year of recent home health OT experience; field-based experience strongly preferred
  • OASIS assessment experience required
  • Proficiency with Home Care Home Base (HCHB) preferred
  • Excellent clinical, organizational, and communication skills
  • Reliable transportation, valid drivers license, and auto insurance
