A skilled and compassionate occupational therapy professional is sought to deliver high-quality care to patients in a home health setting. This contract-based role focuses on empowering individuals to improve their daily living skills, maximize independence, and ensure safety at home. The position best suits someone who thrives in a self-directed, field-based environment and is dedicated to achieving the highest patient outcomes.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
- Full-time, 13-week contract
Qualifications:
- Valid, active Occupational Therapist (OT) license in the state of practice
- At least 1 year of recent experience in Home Health Occupational Therapy (field experience preferred)
- Working knowledge of OASIS documentation required
- Proficiency with Home Care Home Base (HCHB) preferred
- Excellent clinical assessment skills and functional training expertise
- Reliable transportation with valid driver's license and current auto insurance
- Strong organization, time management, and communication abilities
- Ability to manage a flexible, independent field schedule
Primary Responsibilities:
- Complete comprehensive assessments, including OASIS, for new admissions, resumptions, recertifications, and discharge when needed
- Develop and implement individualized treatment plans targeting ADLs, IADLs, upper extremity function, cognition, and home safety goals
- Deliver skilled interventions such as therapeutic activities, self-care and energy conservation training, adaptive equipment instruction, and environmental modifications
- Recommend and educate on appropriate durable medical equipment (DME)
- Regularly reassess patient progress and adjust plans of care accordingly
- Maintain timely, accurate documentation within Home Care Home Base (HCHB)
- Collaborate with nurses, therapists, social workers, and other team members to coordinate patient care
- Attend case conferences and participate in continual professional development
- Ensure compliance with agency, regulatory, and Medicare guidelines
