A rewarding opportunity is available for a skilled and compassionate Home Health Occupational Therapist to provide specialized care in patients homes throughout Colorado Springs. This full-time contract position offers a dynamic setting where you will directly impact patients independence, safety, and quality of life through expert occupational therapy interventions.

Key Responsibilities:
- Perform comprehensive occupational therapy evaluations, including OASIS assessments, tailored to home health settings
- Develop and implement individualized treatment plans focusing on ADLs, IADLs, cognition, upper extremity function, and home safety
- Administer skilled therapeutic interventions such as adaptive equipment training, energy conservation, and environmental modifications
- Collaborate closely with an interdisciplinary team to ensure coordinated patient care and optimal outcomes
- Educate patients and caregivers on functional independence, fall prevention, and compensatory strategies
- Maintain accurate, timely documentation using Home Care Home Base (HCHB) electronic records system
- Adhere to regulatory standards, agency policies, and Medicare home health guidelines
Qualifications & Experience:
- Current and active Occupational Therapist license in Colorado, in good standing
- Minimum 1 year recent experience in home health occupational therapy; field-based experience preferred
- Proficient with OASIS documentation and Home Care Home Base (HCHB) software preferred
- Strong clinical assessment and patient education skills
- Reliable transportation with valid drivers license and auto insurance
- Self-directed with excellent communication, time management, and organizational abilities
Schedule & Contract Details:
- Contract type: Full-time, 13-week duration
- Schedule: Monday through Friday, 8:00 AM to 5:00 PM
- Caseload: Approximately 2532 points weekly (57 patient visits daily)
- Daily travel within assigned territory with mileage reimbursement
