Job Description

Deliver high-quality, compassionate care as a Home Health Occupational Therapist, providing essential therapy services to patients in their homes. This full-time, 13-week contract role focuses on improving patient independence, safety, and quality of life by addressing their activities of daily living and environmental needs.

Schedule:

  • Monday Friday, 8:00 AM 5:00 PM
  • Field-based, visiting patient homes within a designated territory

Key Responsibilities:

  • Conduct comprehensive OT evaluations, utilizing OASIS assessments as required for care transitions
  • Create and implement tailored plans focusing on ADLs, IADLs, upper extremity function, cognition, and home safety
  • Provide skilled OT interventions, including self-care training, therapeutic activities, adaptive equipment training, and home modifications
  • Assess and recommend appropriate durable medical equipment (DME)
  • Track and report progress, adjusting care plans as needed and communicating with caregivers and interdisciplinary team members
  • Educate patients and families on functional independence, safety awareness, and the use of adaptive strategies
  • Complete accurate and timely documentation using Home Care Home Base (HCHB)
  • Collaborate with nurses, PTs, SLPs, medical social workers, and aides to deliver coordinated, patient-centered care
  • Participate in professional development activities and maintain compliance with regulatory standards

Qualifications:

  • Valid Occupational Therapist license in good standing
  • Minimum one year of recent Home Health OT experience (field-based experience preferred)
  • Proficiency with OASIS and familiarity with HCHB
  • Exceptional clinical assessment, training, and organizational skills
  • Reliable transportation, valid drivers license, and proof of insurance
  • Ability to work autonomously in a flexible, travel-based role
