Job Description

Advance your career as a Home Health Occupational Therapist, using your clinical expertise to make a difference in patients lives by delivering compassionate, skilled occupational therapy directly in their homes. This 13-week contract offers you the chance to work with a diverse patient population, guiding individuals toward greater independence and functional improvement within the comfort of their own environment.

Schedule:

  • Monday Friday, 8:00 AM 5:00 PM

Key Responsibilities:

  • Perform comprehensive occupational therapy evaluations, including OASIS assessments for required patient admissions, recertifications, resumptions, and discharges
  • Develop and implement individualized plans of care tailored to functional goals such as ADLs, IADLs, upper extremity rehabilitation, cognition, and home safety
  • Deliver skilled interventions such as therapeutic activities, adaptive equipment training, self-care activities, energy conservation education, and environmental modifications
  • Assess and recommend durable medical equipment as appropriate for patient needs
  • Monitor patient progress, adjust treatment plans accordingly, and provide timely updates to physicians and the care team
  • Document care using the Home Care Home Base (HCHB) electronic system
  • Collaborate closely with nurses, PT, SLP, social workers, and aides to ensure coordinated, high-quality care delivery
  • Participate in interdisciplinary meetings, professional development, and maintain regulatory compliance

Qualifications:

  • Active Occupational Therapist license in good standing for the state
  • At least 1 year of recent home health OT experience required; strong preference for those with field-based backgrounds
  • Experience with Home Care Home Base and OASIS assessments is required
  • Reliable transportation, valid drivers license, and current auto insurance
  • Excellent communication, clinical assessment, organizational, and independent problem-solving skills
