Job Description

This contract occupational therapy opportunity is designed for a motivated practitioner who excels independently and is committed to helping individuals increase safety and independence in their daily routines.

Schedule:

  • Monday Friday, 8:00 AM 5:00 PM

Key Responsibilities:

  • Perform comprehensive occupational therapy evaluations and OASIS assessments as needed.
  • Develop and implement individualized care plans targeting ADLs, upper extremity function, cognition, and environmental safety.
  • Deliver skilled interventions: therapeutic activities, ADL training, adaptive equipment instruction, and home environment modifications.
  • Assess, recommend, and instruct on durable medical equipment.
  • Monitor and document patient progress, adjusting treatment plans in collaboration with an interdisciplinary team.
  • Counsel patients and caregivers on safety, fall prevention, and maximizing independence.
  • Complete timely, accurate documentation in Home Care Home Base (HCHB).
  • Engage in case conferences, interdisciplinary meetings, and ongoing professional growth activities.
  • Ensure compliance with all agency policies and regulatory guidelines.

Desired Qualifications:

  • Active Occupational Therapist (OT) license in good standing for the state of practice.
  • Minimum 1 year of recent experience in home health occupational therapy.
  • Experience with Home Care Home Base (HCHB) and OASIS assessments required.
  • Strong clinical, assessment, and training skills.
  • Reliable transportation, valid drivers license, and current auto insurance required.
  • Excellent communication, organizational, and time management abilities.
  • Self-directed, with a proven ability to work independently in a field-based environment.
