Job Description
Deliver expert occupational therapy services in patients' homes as a Home Health Occupational Therapist (OT), focused on enhancing independence and safety through tailored interventions. This full-time, 13-week contract role involves providing high-quality care in a field-based setting within the Burlington, NC area.
Responsibilities:
- Perform comprehensive OT evaluations, including OASIS assessments (Start of Care, Resumption, Recertification, Discharge).
- Develop and implement individualized plans addressing ADLs, IADLs, upper extremity function, cognition, and home safety.
- Deliver skilled interventions such as therapeutic activities, adaptive equipment training, energy conservation, and environmental modifications.
- Recommend and assess needs for durable medical equipment.
- Monitor patient progress, adjusting treatment plans and communicating updates with physicians and interdisciplinary teams.
- Educate patients and caregivers on fall prevention, safety, functional independence, and compensatory strategies.
- Maintain accurate, timely documentation within Home Care Home Base (HCHB) system.
- Collaborate with nurses, PTs, SLPs, social workers, and home health aides to ensure coordinated care.
- Participate in case conferences, interdisciplinary meetings, and pursue professional development.
- Adhere to agency policies, regulatory standards, and Medicare home health guidelines.
Qualifications:
- Active OT license in good standing within state of practice.
- At least 1 year of recent home health OT experience; field-based experience preferred.
- Experience with OASIS assessments required; proficiency in Home Care Home Base (HCHB) preferred.
- Strong clinical assessment and functional training skills.
- Reliable transportation, valid drivers license, and auto insurance.
- Ability to work independently and manage a flexible field schedule.
- Excellent communication, organization, and time management skills.
Schedule:
- Monday to Friday, 8:00 AM 5:00 PM
- Caseload of approximately 2532 points weekly (57 patient visits daily)
