Exciting opportunity available for a dedicated Home Health Occupational Therapist eager to make a direct impact in the lives of patients. In this field-based position, you'll deliver high-quality occupational therapy services right in patients homes, focusing on boosting their independence and enhancing their safety with tailored, compassionate care.
Schedule/Hours:
- Monday Friday, 8:00 AM 5:00 PM
- Full-time, 13-week contract
Key Qualifications:
- Active Occupational Therapist (OT) license in good standing
- At least 1 year of recent experience in Home Health Occupational Therapy; field experience strongly preferred
- OASIS assessment experience required
- Familiarity with Home Care Home Base (HCHB) electronic documentation preferred
- Solid clinical assessment skills and expertise in delivering functional training
- Must have reliable transportation, valid drivers license, and auto insurance
- Demonstrated ability to work independently, stay organized, and communicate effectively
Primary Responsibilities:
- Conduct comprehensive OT evaluations in patients homes, including OASIS assessments for admit, resume, recertification, and discharge visits
- Develop and implement individualized care plans with a focus on ADLs, IADLs, upper extremity function, cognition, and home safety
- Deliver skilled interventions such as therapeutic activities, adaptive equipment training, energy conservation, and environmental modifications
- Collaborate closely with nurses, physical therapists, speech therapists, and social workers to ensure coordinated patient care
- Provide training and education to patients and caregivers on adaptive strategies and home safety
- Maintain accurate, timely documentation in Home Care Home Base (HCHB)
- Participate in case conferences, meetings, and ongoing professional development
