Job Description

Provide essential, life-changing care in a field-based role supporting patients safety and independence. As a Home Health Occupational Therapist, youll deliver therapy services directly in patients homes, helping to maximize their ability to perform daily activities and promoting healthier, safer living environments.

Contract Overview:

  • Full-time, 13-week contract
  • Monday Friday, 8:00 AM 5:00 PM
  • Typically 57 patient visits daily (approx. 2532 points weekly)
  • Travel within assigned territory (mileage reimbursement provided)
  • Use of Home Care Home Base (HCHB) electronic documentation and OASIS assessment tool

Key Responsibilities:

  • Complete thorough occupational therapy evaluations, including OASIS assessments for Start of Care, Resumption, Recertification, and Discharge as needed
  • Design individualized care plans focused on ADLs, IADLs, upper extremity function, safety, and cognition
  • Deliver targeted interventions: self-care training, adaptive equipment assessment and training, energy conservation, home modifications, and safety education
  • Track progress, update care plans as appropriate, and communicate with physicians and care team members
  • Guide patients and caregivers on strategies for functional independence, injury prevention, and optimal use of recommended DME
  • Maintain accurate, timely documentation in HCHB per all regulatory guidelines
  • Participate in interdisciplinary case conferences and ongoing professional development

Qualifications:

  • Current Occupational Therapist license (state of assignment), in good standing
  • At least one year of recent home health OT experience
  • Proficiency with Home Care Home Base (HCHB) strongly preferred
  • OASIS experience required
  • Solid clinical assessment, problem-solving, and organizational skills
  • Reliable transportation, valid drivers license, and proof of insurance
  • Strong independence and adaptability for a field-based setting
