Job Description

A skilled Home Health Occupational Therapist (OT) is needed to provide compassionate, patient-centered care in the comfort of patients' homes. This role focuses on enhancing patients ability to perform activities of daily living (ADLs), promoting independence, and ensuring home safety through tailored therapeutic interventions. The position involves comprehensive evaluations, individualized treatment plans, and collaboration with interdisciplinary care teams to achieve optimal outcomes.

Responsibilities:

  • Conduct thorough occupational therapy evaluations, including OASIS assessments.
  • Develop and implement customized care plans targeting ADLs, IADLs, cognition, and safety.
  • Deliver skilled interventions such as therapeutic activities, self-care training, and adaptive equipment instruction.
  • Recommend and assess adaptive and durable medical equipment needs.
  • Monitor patient progress and adjust care plans accordingly.
  • Educate patients and caregivers on independence, fall prevention, and safety strategies.
  • Complete precise documentation using Home Care Home Base (HCHB).
  • Collaborate with an interdisciplinary team including nurses, PTs, SLPs, and social workers.
  • Participate in case conferences and ensure compliance with Medicare and agency policies.

Qualifications/Desired Experience:

  • Active Occupational Therapist (OT) license in good standing within the state.
  • Minimum one year of recent Home Health OT experience; field experience preferred.
  • Proficiency with Home Care Home Base (HCHB) software preferred.
  • Experience with OASIS assessments required.
  • Strong clinical, communication, and organizational skills.
  • Reliable transportation, valid drivers license, and auto insurance.
  • Ability to work independently in a flexible, field-based setting.

Job Details:

  • Contract type: Full-time, 13-week duration.
  • Schedule: Monday Friday, 8:00 AM 5:00 PM.
  • Caseload: Approximately 2532 points per week (57 patient visits daily).
  • Travel: Daily travel to patient homes within assigned territory; mileage reimbursed.
