Job Description

Deliver compassionate care and make a meaningful impact as a Home Health Occupational Therapist, supporting patients independence and safety within their own homes. This contract opportunity offers a dynamic field-based environment where you will directly enhance quality of life by helping clients improve their ability to perform activities of daily living and navigate daily routines more confidently.

Schedule:

  • Monday Friday, 8:00 AM 5:00 PM
  • Full-time, 13-week contract
  • Expected caseload: Approximately 2532 points per week (typically 57 patient visits per day, based on visit types)

Key Responsibilities:

  • Conduct comprehensive OT evaluations, incorporating OASIS assessments as needed.
  • Develop and execute individualized care plans tailored to ADLs, upper extremity function, cognition, and safety in the home.
  • Deliver skilled interventions including therapeutic activities, self-care and energy conservation training, and environmental modifications.
  • Educate patients and caregivers regarding adaptive strategies, home safety, and equipment use.
  • Monitor progress, adapt plans to evolving needs, and coordinate closely with physicians and interdisciplinary team members.
  • Maintain precise and timely documentation using Home Care Home Base (HCHB).
  • Participate in collaborative interdisciplinary meetings and ongoing professional development.
  • Comply with all regulatory guidelines and agency policies.

Desired Experience & Credentials:

  • Active Occupational Therapist license (state-specific, in good standing)
  • At least 1 year of recent field-based Home Health OT experience
  • OASIS assessment and Home Care Home Base (HCHB) proficiency (preferred)
  • Demonstrated clinical assessment strengths and organizational skills
  • Ability to independently manage a flexible, travel-based schedule
  • Valid drivers license, reliable transportation, and current auto insurance
