Job Description

Deliver exceptional patient care and support independence as a Home Health Occupational Therapist working in the field. This contract position allows you to bring critical rehabilitation services directly to patients in their homes, emphasizing rehabilitation, safety, adaptive skills, and overall well-being. You will play a vital role in helping individuals reach their highest level of functional independence in a setting that matters mosttheir home.

Schedule:

  • Monday Friday, 8:00 AM 5:00 PM
  • 13-week contract
  • Caseload: Approximately 2532 points per week (typically 57 home visits per day depending on visit types)

Key Responsibilities:

  • Conduct comprehensive occupational therapy evaluations in patient homes, including OASIS assessments when required.
  • Develop, implement, and adjust individualized plans of care focused on ADLs, IADLs, upper extremity function, cognition, and safety.
  • Deliver skilled interventions such as therapeutic activities, self-care training, adaptive equipment recommendations, and home modifications.
  • Monitor progress, update care plans, and communicate effectively with the interdisciplinary care team and physicians.
  • Educate patients and caregivers on adaptive strategies, fall prevention, safety protocols, and the use of durable medical equipment.
  • Ensure accurate, timely documentation using Home Care Home Base (HCHB).
  • Collaborate in case conferences and professional development activities.

Qualifications:

  • Active Occupational Therapist (OT) license (state of practice, in good standing)
  • At least 1 year of recent field-based Home Health OT experience
  • OASIS assessment proficiency required
  • HCHB experience strongly preferred
  • Reliable transportation, valid drivers license, and auto insurance
  • Independent, organized, and clear communicator
