Job Description

Bring your expertise and compassion to the forefront as a Home Health Occupational Therapist, where your skills will have a direct impact on patient independence and quality of life. In this field-based contract role, you'll be an integral part of an interdisciplinary care team, providing hands-on occupational therapy services to patients in their homes while supporting optimal outcomes and safety.

Schedule:

  • Monday through Friday, 8:00 AM 5:00 PM
  • 13-week contract assignment
  • Expect a caseload of approximately 2532 points per week (about 57 patient visits per day)
  • Daily local travel to patient homes within your assigned territory

What Were Looking For:

  • Current and active Occupational Therapist (OT) license in state of practice, in good standing
  • At least one year of recent home health occupational therapy experience
  • OASIS assessment experience required
  • Prior use of Home Care Home Base (HCHB) electronic documentation preferred
  • Reliable transportation, valid drivers license, and up-to-date auto insurance
  • Strong clinical assessment, care planning, and communication skills
  • Independent, organized, and comfortable working in the field

Core Responsibilities:

  • Provide comprehensive home-based OT evaluations (including OASIS assessments), develop and implement individualized treatment plans
  • Support patients improvement in ADLs/IADLs, upper extremity function, cognition, and home safety
  • Deliver skilled interventions: therapeutic activities, adaptive equipment training, energy conservation, and home modifications
  • Educate patients and caregivers on safety, independence, and the use of assistive devices
  • Collaborate with nurses, PTs, SLPs, social workers, and aides for coordinated care
  • Maintain accurate and timely electronic documentation
  • Attend case conferences and interdisciplinary meetings as required
