A rewarding per diem opportunity is available for an experienced Nurse Practitioner to provide home health risk assessments for Medicare Advantage plan members throughout Transylvania County, North Carolina. Schedule:
- Flexible shifts available Monday through Sunday
- Set your own schedule according to your availability
- Minimum weekly or bi-weekly hourly commitment required
Key Responsibilities:
- Conduct annual home health risk assessments for Medicare Advantage plan members in their homes
- Evaluate patients prior and current medical history, existing conditions, and medication history during approximately 45-minute visits
- Offer patient education focused on health conditions and medications
- Identify gaps in care and make recommendations to ensure comprehensive patient support
- No requirement for treatment plan creation or prescribing medicationfocus remains on assessments and education
- Complete assessments in a variety of neighborhoods in Transylvania County, including Brevard and Rosman
Qualifications:
- Active Nurse Practitioner license in North Carolina
- Current CPR certification (BLS)
- Board Certification via AANP or ANCC
- Minimum of 6 months current NP experience preferred
- MSN degree from an accredited nursing program
Compensation for this role is $80-$90 per evaluation, along with a mileage stipend and compensation for administrative responsibilities.
