Deliver proactive patient care and make a meaningful impact as a Nurse Practitioner with per diem flexibility in the home health setting. Leverage your expertise to perform comprehensive risk assessments for Medicare Advantage plan members in Oklahoma County, including Oklahoma City, Edmond, and Midwest City. Shape your ideal schedule by selecting shifts between Monday and Sunday, with the ability to set your own minimum weekly or bi-weekly hours.
Schedule/Hours:
- Flexible shifts available seven days a week
- Choose your own schedule
- Minimum weekly or bi-weekly hourly commitment required
Key Responsibilities:
- Conduct in-home health risk assessments (approximately 45 minutes) for Medicare Advantage members
- Review and evaluate medical history, current conditions, and medication use
- Educate patients about their health conditions and medications
- Identify care gaps and recommend appropriate next steps to support patient outcomes
- No responsibility for creating treatment plans or prescribing medications
Qualifications:
- Active Nurse Practitioner license in the State of Oklahoma
- Board Certification via AANP or ANCC
- MSN from an accredited nursing school
- Current CPR certification (BLS)
- At least 6 months of recent NP experience (preferred)
- Home health experience, particularly with Medicare Advantage members, is valued
