Schedule:
- Flexible shifts available between Monday and Sunday
- Create your own schedule
- Minimum weekly or bi-weekly hourly commitment required
Responsibilities:
- Perform comprehensive annual home health risk assessments for Medicare Advantage plan members
- Review and evaluate prior/current medical history, chronic conditions, and medication regimens
- Educate patients on their medical conditions and medications to promote health literacy
- Identify gaps in care and recommend solutions where necessary
- Visits typically last about 45 minutes and may involve annual wellness or post-discharge assessments
- No requirement for treatment planning or medication prescribing
Qualifications:
- Active Nurse Practitioner license in Oklahoma
- MSN from an accredited nursing program
- Board certified through AANP or ANCC
- Current CPR (BLS) certification
- Minimum of 6 months Nurse Practitioner clinical experience preferred
This position involves traveling to the residences of members in Oklahoma City and nearby areas, including Edmond and Midwest City within Oklahoma County.
