Elevate your career as a Nurse Practitioner specializing in home health by performing meaningful risk assessments for Medicare Advantage members throughout Columbiana County, OH. In this role, you will directly engage with patients in their homes, providing high-impact annual or post-discharge assessments. Each visit averages 45 minutes and could take place in areas such as Perry Township, Salem, or East Liverpool.
Schedule:
- Flexible shifts available any day between Monday and Sunday
- Option to create your own schedule
- Minimum weekly or bi-weekly hourly commitments
Key Responsibilities:
- Carry out comprehensive home health risk assessments for Medicare Advantage plan members
- Assess patients prior and current medical history, ongoing conditions, and medication history during each evaluation
- Deliver clear education to patients regarding their medical conditions and medications
- Identify care gaps and communicate actionable recommendations to improve patient health outcomes
- Focus exclusively on assessmentsno prescribing or development of treatment plans required
Qualifications:
- Current Nurse Practitioner license in Ohio
- Valid CPR certification (BLS)
- Board Certification via AANP or ANCC
- At least 6 months of recent NP experience preferred
- MSN degree from an accredited program
