Experienced Nurse Practitioners have an opportunity to deliver impactful, patient-centered care by performing home health risk assessments for Medicare Advantage members. This per diem role encompasses conducting thorough evaluations for members in their residences, focusing on Hampton, VA. Each visit is designed as a comprehensive assessmentlasting approximately 45 minutesand may constitute an annual health review or a post-discharge check-in.
Schedule:
- Flexible shifts available seven days a week
- Ability to set your own work schedule
- Choose your minimum weekly or bi-weekly hourly commitment
Core Responsibilities:
- Carry out in-home annual risk assessments for Medicare Advantage plan participants
- Review health and medication histories for accuracy and completeness
- Assess prior and current medical conditions
- Educate patients on their conditions, care plans, and medication regimens
- Identify gaps in care, offering recommendations to improve patient outcomes
- No requirement to create formal treatment plans or prescribe medications
Qualifications:
- Valid Nurse Practitioner license in Virginia
- Graduate of an accredited school with an MSN degree
- Current Board Certification (AANP or ANCC)
- Active BLS CPR certification
- Minimum of 6 months current NP experience preferred
- Excellent communication and organizational skills
