Embark on a rewarding per diem opportunity as a Nurse Practitioner specializing in home health assessments across Henry County, IN. This flexible position allows for self-scheduling with shifts available any day between Monday and Sunday, empowering you to balance professional and personal commitments with ease. A minimum weekly or bi-weekly hourly commitment is required, and you will visit Medicare Advantage plan members in their homes in communities such as New Castle, Middletown, and Knightstown.

Key Responsibilities:

• Conduct comprehensive annual home health risk assessments for Medicare Advantage members
• Evaluate patient medical histories, existing conditions, and medication regimens
• Educate patients on their health conditions and medication management
• Identify gaps in care and offer recommendations for follow-up
• Focus on evaluationno need to create treatment plans or prescribe medications

Desired Qualifications:

• Active Nurse Practitioner license within Indiana
• Board Certification via AANP or ANCC
• CPR (BLS) certification, current and valid
• Graduate from an accredited MSN program
• Minimum six months of clinical Nurse Practitioner experience preferred
