Job Description
As a Nurse Practitioner with expertise in home health, you will play a pivotal role by conducting risk assessments for Medicare Advantage plan members. This per diem opportunity offers flexibility and autonomy, allowing you to design your own work schedule within Delaware County, IN. You will be scheduled across various areas, including Muncie, Yorktown, and Chesterfield, completing assessments in patients homes.
Schedule:
- Flexible shifts available Monday through Sunday
- Self-scheduling to meet personal and professional needs
- Minimal weekly or bi-weekly commitment required
Responsibilities:
- Complete comprehensive home health risk assessmentseach visit lasts approximately 45 minutes
- Assess prior/current medical history, chronic conditions, and medication regimen
- Educate members on health conditions, disease management, and appropriate medication usage
- Identify care gaps and provide clinical recommendations without treating or prescribing medications
- Document findings in accordance with compliance and company protocols
Desired Qualifications:
- Active Nurse Practitioner license in the state of Indiana
- Board certification through AANP or ANCC
- Current BLS certification (CPR)
- Graduate of an accredited MSN program
- At least 6 months of recent NP experience (home health preferred)
