Job Description

Step into a per diem opportunity that offers flexibility and meaningful patient interactions by providing advanced home health assessments within Delaware County, Indiana. As a Nurse Practitioner specializing in Home Health, you will perform comprehensive risk evaluations for Medicare Advantage plan members residing in areas such as Center Township and the surrounding communities of Muncie, Yorktown, and Chesterfield.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Design your own work schedule
  • Choose minimum hourly commitments on a weekly or bi-weekly basis

Key Responsibilities:

  • Perform annual home health risk assessments during 45-minute visits
  • Review and document patient medical history, current conditions, and medication use
  • Deliver education on chronic and acute conditions, as well as medication regimens
  • Identify care gaps and present recommendations for patient support and follow-up
  • Visits may include annual wellness checks or post-discharge assessments
  • No treatment planning, prescribing, or follow-up management required

Qualifications:

  • Active Nurse Practitioner license in Indiana
  • Board Certified through AANP or ANCC
  • Current BLS/CPR certification
  • Masters degree in Nursing from an accredited institution
  • At least six months of direct NP experience preferred
  • Proficiency in independently assessing patients and documenting findings

Your commitment and expertise will be recognized with compensation ranging from $80 to $90 per completed evaluation along with a mileage stipend and pay for administrative time.
