Step into a dynamic role conducting essential home health risk assessments throughout Yonkers and Westchester County, NY. This per diem opportunity is designed for Nurse Practitioners dedicated to delivering high-quality, patient-focused care in a home health setting. Assessments are performed on Medicare Advantage plan members and may include annual evaluations or post-discharge visits, with each assessment averaging 45 minutes in duration. Locations for visits may include New Rochelle, Mount Vernon, White Plains, and other neighboring areas within Westchester County.
Schedule:
- Choose your shifts: Flexible scheduling available between Monday and Sunday
- Control your own calendar: Build your own schedule
- Minimum weekly or bi-weekly hourly commitment required
Responsibilities:
- Complete detailed home health risk assessments for Medicare Advantage members
- Evaluate current and historical medical conditions, medication regimens, and prior treatment
- Provide patient education on health conditions and prescribed medications
- Identify and document care gaps, delivering recommendations for improved health outcomes
- No need to develop treatment plans or prescribe medications
Qualifications:
- Active Nurse Practitioner License in New York
- MSN degree from an accredited program
- Current CPR certification (BLS)
- Board Certification via AANP or ANCC
- At least 6 months of recent NP experience preferred
