Step into a dynamic per diem opportunity as a Nurse Practitioner specializing in home health risk assessments. This role serves Medicare Advantage plan members throughout Yonkers and the broader Westchester County areaincluding New Rochelle, Mount Vernon, and White Plains. Assessments typically last 45 minutes and may range from routine annual evaluations to post-discharge visits, with the flexibility to create your own schedule.

Schedule:

• Flexible shifts available seven days a week
• Craft your preferred schedule to meet personal and professional commitments
• Minimum weekly or bi-weekly hourly commitment required

Key Responsibilities:

• Conduct in-home annual risk assessments for a Medicare Advantage plan
• Review and document prior and current medical history, conditions, and medication regimens
• Provide education to members regarding health conditions and prescribed medications
• Detect care gaps and suggest actionable recommendations
• No development of treatment plans or medication prescribing required

Qualifications:

• Active Nurse Practitioner license in New York
• Current BLS (CPR) certification
• Board certification through AANP or ANCC
• At least 6 months of recent Nurse Practitioner experience preferred
• Master of Science in Nursing (MSN) from an accredited institution

This per diem position offers competitive earnings of $80$90 per evaluation, plus a mileage stipend and compensation for administrative time.
