Advance your practice as a Nurse Practitioner providing home health risk assessments to Medicare Advantage plan members in Yonkers and across Westchester County, NY. In this per diem role, you'll have full flexibility to create your own schedule, with assignments available in key areas such as New Rochelle, Mount Vernon, and White Plains.
Schedule:
- Flexible shifts available Monday through Sunday
- Set your own workdays and hours
- Choose minimum weekly or bi-weekly commitments to fit your lifestyle
Key Responsibilities:
- Perform thorough annual health risk evaluations in patients homes
- Assess prior and current medical and medication history
- Educate patients on chronic conditions and ongoing medication management
- Identify care gaps and make recommendations to help improve care quality
- No prescribing or treatment plans required
Qualifications:
- Active Nurse Practitioner license in New York
- Board certification through AANP or ANCC
- MSN degree from an accredited nursing program
- Current CPR/BLS certification
- At least 6 months of Nurse Practitioner experience preferred
This opportunity offers a per-assessment earning structure of $80$90 for each evaluation completed, plus a mileage stipend and compensation for administrative time.
