Bring your clinical expertise and patient-focused care to a per diem opportunity as a Nurse Practitioner specializing in Home Health risk assessments. In this role, you will travel throughout Yakima County, including areas such as Yakima, Sunnyside, and Grandview, conducting comprehensive risk assessments for Medicare Advantage plan members in the comfort of their own homes.

Schedule:

• Flexible shifts available Monday through Sunday
• Design your own schedule
• Set minimum weekly or bi-weekly hourly commitments

Key Responsibilities:

• Perform annual home health risk assessments with each evaluation averaging approximately 45 minutes.
• Assess prior and current medical history, document medical conditions, and review medication usage.
• Educate members on their medical conditions and prescribed medications.
• Identify care gaps and suggest actionable recommendations for improving patient outcomes.
• Conduct both annual health assessments and post-discharge visits.
• No requirement for creating treatment plans or prescribing medications.

Qualifications:

• Active Nurse Practitioner license in Washington State
• Current BLS (CPR) certification
• Board certified through AANP or ANCC
• MSN degree from an accredited nursing school
• At least six months of recent Nurse Practitioner experience preferred
