Job Description

Deliver high-quality, patient-centered care as a Nurse Practitioner conducting home health risk assessments across Wythe County. This per diem position is ideal for professionals seeking autonomy and flexibility, as well as the opportunity to positively impact Medicare Advantage plan members living in Wytheville, Rural Retreat, and the surrounding areas.

Schedule:

  • Flexible shifts available between Monday and Sunday
  • Create your own work schedule
  • Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

  • Conduct comprehensive home health risk assessments, including annual evaluations and post-discharge visits
  • Review and assess prior and current medical history, existing medical conditions, and medication history
  • Provide patient education related to medical conditions and proper medication usage
  • Identify gaps in care; offer recommendations to improve patient outcomes
  • Administrative tasks related to documentation and visit reporting
  • No requirement to develop treatment plans or prescribe medications

Qualifications:

  • Active Nurse Practitioner License in Virginia
  • Board Certification from AANP or ANCC
  • Current CPR certification (BLS)
  • MSN degree from an accredited nursing program
  • At least 6 months of current Nurse Practitioner experience preferred
