Job Description

Experienced Nurse Practitioners are needed to perform home health risk assessments on Medicare Advantage members throughout Kent County, MIincluding Grand Rapids, Wyoming, and Kentwood. This per diem opportunity offers the flexibility to design your own schedule, accommodating work-life balance with shifts available any day of the week.

Schedule/Hours:

  • Flexible shifts available Monday through Sunday
  • Create your own schedule to fit your availability
  • Minimum weekly or bi-weekly hourly commitment required

Key Responsibilities:

  • Conduct thorough annual risk assessments in patients homes
  • Evaluate current and prior medical history, diagnoses, and medication use
  • Educate patients on medical conditions and medications as needed
  • Identify gaps in care and provide practical recommendations
  • No requirement to create treatment plans or prescribe medications

Qualifications:

  • Active Nurse Practitioner license in Michigan
  • Current CPR (BLS) certification
  • Board Certification (AANP or ANCC)
  • MSN degree from an accredited nursing program
  • At least 6 months of recent NP experience preferred

Per evaluation, practitioners will receive $80-$90, including a stipend for mileage and administrative time, making this not only flexible but financially rewarding.
