Key Responsibilities:
- Conduct annual health risk assessments for Medicare Advantage plan members in patients homes throughout St. Francis County, AR.
- Carefully evaluate patients prior and current medical history, ongoing medical conditions, and medication usage during approximately 45-minute visits.
- Provide clear education regarding medical conditions and medication regimens.
- Identify gaps in care and offer recommendations to improve patient outcomes.
- Visits may include annual health assessments or post-discharge follow-up, as assigned.
- No requirement for creating treatment plans or prescribing medications.
Qualifications:
- Active Nurse Practitioner license in Arkansas
- Current CPR certification (BLS)
- Board Certification through AANP or ANCC
- At least 6 months of current Nurse Practitioner experience preferred
- MSN degree from an accredited nursing school
