Schedule:

• Flexible shifts available seven days a week

• Minimum weekly or bi-weekly hourly commitment

Responsibilities:

• Complete comprehensive home health risk assessments for Medicare Advantage members
• Assess patients prior and current medical history, health conditions, and medication usage
• Educate members about their medical conditions and medications in a home setting
• Identify any gaps in care and recommend appropriate follow-up actions
• Assist with varied patient visits, such as annual assessments or post-discharge check-ins
• No prescribing or treatment plan developmentfocus on assessment, education, and care coordination

Qualifications:

• Active Nurse Practitioner license in Arkansas
• Board Certification (AANP or ANCC)
• Minimum of 6 months recent Nurse Practitioner experience preferred
• MSN degree from an accredited nursing program
• Current BLS (CPR) certification
