Job Description

Key Responsibilities:

  • Conduct annual health risk assessments for Medicare Advantage plan members in patients homes throughout St. Francis County, AR.
  • Carefully evaluate patients prior and current medical history, ongoing medical conditions, and medication usage during approximately 45-minute visits.
  • Provide clear education regarding medical conditions and medication regimens.
  • Identify gaps in care and offer recommendations to improve patient outcomes.
  • Visits may include annual health assessments or post-discharge follow-up, as assigned.
  • No requirement for creating treatment plans or prescribing medications.

Qualifications:

  • Active Nurse Practitioner license in Arkansas
  • Current CPR certification (BLS)
  • Board Certification through AANP or ANCC
  • At least 6 months of current Nurse Practitioner experience preferred
  • MSN degree from an accredited nursing school

You will receive between $80 and $90 per evaluation, along with a mileage stipend and compensation for administrative time. This arrangement allows you to maximize your earnings on a flexible schedule, fitting shifts around other professional or personal commitments.
