Enhance the quality of patient care by providing vital home health risk assessments for Medicare Advantage members. This per diem opportunity empowers nurse practitioners to make a significant impact within Laurens County, GAincluding areas such as Dublin, East Dublin, and Dudleyby supporting patient health in the comfort of their own homes.
Schedule:
- Flexible shifts available Monday through Sunday
- Design your own schedule
- Opportunity for minimum weekly or bi-weekly hourly commitments
Responsibilities:
- Perform comprehensive home health risk assessments, averaging 45 minutes per visit
- Assess prior and current medical history, existing medical conditions, and medication use
- Deliver patient education regarding diagnoses and medications
- Identify care gaps and provide actionable recommendations for improved health outcomes
- Focus strictly on assessment and educationno responsibility for prescribing or treatment planning
Qualifications:
- Active Nurse Practitioner license in Georgia
- Current CPR certification (BLS)
- Board Certification through AANP or ANCC
- MSN degree from an accredited nursing program
- At least 6 months of recent nurse practitioner experience preferred
