Job Description
Seeking a Nurse Practitioner for specialized home health risk assessment visits. This per diem opportunity is ideal for experienced NPs interested in flexible, self-managed scheduling and meaningful patient interactions. Shifts are available throughout Kent County, with visits to areas including Warwick, West Warwick, and Coventry.
Schedule:
- Flexible shifts offered any day of the week (Monday through Sunday)
- Self-directed schedulingchoose the days and times that work for you
- Minimum weekly or bi-weekly hour commitment required
Key Responsibilities:
- Conduct comprehensive annual risk assessments for Medicare Advantage plan members in their homes
- Review previous and current medical and medication histories with each patient
- Deliver clear education about existing medical conditions and current medications
- Identify care gaps and provide documented recommendations for follow-up
- Focus solely on assessment and educationno responsibility for prescribing medications or developing long-term treatment plans
Qualifications:
- Active Nurse Practitioner license in Rhode Island (RI)
- Board Certification through AANP or ANCC
- Current Basic Life Support (BLS/CPR) certification
- Master of Science in Nursing from an accredited institution
- At least 6 months of relevant Nurse Practitioner experience preferred
