Deliver compassionate patient care and utilize your clinical skills to make a direct impact through home health risk assessments for Medicare Advantage members in Warren County, VA. This per diem Nurse Practitioner opportunity offers remarkable flexibility, allowing you to create your own schedule for shifts across Monday through Sunday. Assignments will take you into patient homes in areas including Front Royal, Bentonville, and throughout Warren County, where youll provide essential health insights and support.
Schedule:
- Flexible shifts available Monday through Sunday
- Freedom to design your own work schedule
- Minimum weekly or bi-weekly hourly commitment required
Key Responsibilities:
- Conduct annual risk assessments in patient homes on Medicare Advantage plan members
- Review and evaluate current and previous medical history, including conditions and medications
- Educate patients on their diagnoses and drug regimens
- Detect care gaps and recommend steps for care improvement
- No treatment planning or prescription duties included
Required Qualifications:
- Possess a current Nurse Practitioner license in Virginia
- Hold current BLS (CPR) certification
- Board Certified through AANP or ANCC
- MSN from an accredited nursing program
- At least six months experience as an NP strongly preferred
