Experienced Nurse Practitioners passionate about delivering quality care in home health will excel in this dynamic, per diem opportunity. In this role, you will perform home health risk assessments for Medicare Advantage plan members throughout Wythe County. Assessments take about 45 minutes each and may include annual health reviews or post-discharge evaluations. Your visits will be conducted in locations such as Wytheville, Rural Retreat, and surrounding areasexpect varied assignments based on member need.

Schedule:

• Flexible shifts available Monday through Sunday
• Choose your assignments and build your own schedule
• Commit minimum weekly or bi-weekly hours as preferred

Key Responsibilities:

• Complete thorough annual home health risk assessments for Medicare Advantage plan members
• Obtain and evaluate prior/current medical history, health conditions, and medications
• Educate patients about their health conditions and prescriptions
• Recognize gaps in care and offer recommendations for follow-up
• No responsibility for prescribing or creating long-term treatment plans

Qualifications:

• Valid and active Nurse Practitioner license in Virginia
• Board Certification (AANP or ANCC)
• Current CPR certification (BLS)
• MSN from an accredited nursing program
• At least six months NP experience preferred
