Job Description

As a Nurse Practitioner specializing in home health, you'll complete risk assessments for Medicare Advantage plan members throughout Warren County, TN, including areas such as McMinnville, Morrison, and Centertown. Enjoy the flexibility of a per diem role while making meaningful connections in the community.

Schedule:

  • Flexible shifts available seven days a week
  • Design your own work schedule
  • Set your preferred minimum weekly or bi-weekly hours

Key Responsibilities:

  • Complete annual or post-discharge home health risk assessments (average 45 minutes per visit)
  • Evaluate patients medical histories, current health status, and medication lists
  • Educate patients on their health conditions and medications in clear, compassionate terms
  • Recognize and report care gaps, providing actionable recommendations without the need for treatment planning or prescribing
  • Engage with diverse populations across various home settings, ensuring thorough documentation

Qualifications:

  • Valid and unencumbered Nurse Practitioner license in Tennessee
  • Accredited MSN degree in Nursing
  • Board certification through either AANP or ANCC
  • Active CPR certification (BLS)
  • Minimum 6 months of current Nurse Practitioner experience preferred
