Job Description
Advance your healthcare career by providing comprehensive home health risk assessments to Medicare Advantage plan members in the Glynn County, GA region, including Brunswick and the Golden Isles. This per diem opportunity allows nurse practitioners to utilize their expertise while enjoying flexible scheduling designed to accommodate personal and professional commitments.
Schedule:
- Flexible shifts available between Monday and Sunday
- Self-schedulingcreate your own work calendar
- Minimum weekly or bi-weekly hourly commitment required
Primary Responsibilities:
- Perform annual home health risk assessments for members of a Medicare Advantage health plan
- Review and evaluate medical history, conditions, and medication regimens during in-home visits (each lasting approximately 45 minutes)
- Deliver patient education related to medical conditions and medication usage
- Identify gaps in care, offering recommendations for improvement
- No requirement for the development of treatment plans or prescribing medications
Qualifications:
- Active Nurse Practitioner license in the state of Georgia
- Current CPR certification (BLS)
- Board Certification through AANP or ANCC
- Minimum of six months recent experience as an NP preferred
- Master of Science in Nursing (MSN) from an accredited institution
