Make an impact on patient wellness by providing crucial home health risk assessments for Medicare Advantage plan members throughout Le Flore County. This is a per diem opportunity aimed at nurse practitioners seeking flexible scheduling and the ability to shape their own workweek. Shifts are available any day between Monday and Sunday, with the freedom to customize your weekly or bi-weekly hourly commitments.

Responsibilities include:

• Conducting annual and post-discharge home health risk assessments in patients' homes
• Completing comprehensive reviews of prior/current medical history, chronic conditions, and medications
• Educating patients about their health conditions, preventive care, and proper medication usage
• Identifying care gaps and offering actionable recommendations, all without responsibility for treatment planning or prescription writing
• Providing professional, compassionate care that supports patient empowerment and improved outcomes

Desired qualifications:

• Active Nurse Practitioner license in Oklahoma
• Board certification through AANP or ANCC
• Current CPR certification (BLS)
• MSN from an accredited nursing program
• At least 6 months of current NP experience, preferably in home health or primary care

Compensation is offered at $80-$90 per completed evaluation, along with a mileage stipend and administrative pay. Each risk assessment generally requires about 45 minutes, and home visits take place in Poteau, Heavener, Spiro, and additional nearby communities.
